Pipe and Crossing Report:  This report will be filled out for all pipeline crossings and hot taps. Only one crossing per sheet, even if the crossed line are adjacent. This is a very important report and time needs to be taken to insure completion and accuracy. This report is also used for land jobs, so not all blanks will apply. Type in all CAPS, it helps to read the faxes.

Foreman No.: Your name plus the MMI job number. If there is more than one drawing use a hyphen and number sequentially, such as 970711-1.

Area No.: Put the area (no initials) and block number: South Timbalier 211

Date of Occurrence: Date crossing or hot tap made.

City: Offshore, GOM (Gulf of Mexico)

County: NA

State: Federal (or State) Waters

System Check One: Put in the pipeline system; ask MMI if you don't know.

Facility Name/Description: Again, put in the pipeline system

Map/Drawing No.:

Important! Hopefully, you received an alignment map before you left, if not get this information from MMI.

Survey Station:

Important! Put in the coordinates here (X & Y's). Check to see if the Y's have a minus (-) in front of the number. Check with survey to determine Units of Measurement as in UTM or Louisiana South. Place this in Sketch/Description also as a minimum.

Air/Mile Marker: NA

Nominal Dia., W.T. or U.T., Grade, Type:

All this information will be on the Alignment Sheet and will be like:

Corrosion & Coating Information (info on Existing Pipeline): Check all appropriate boxes or state NA if it doesn't apply. For "Coating Type" put CONCRT (concrete) or FBE (fusion bond epoxy) or other correct term. This is for the existing Shell pipeline.

Foreign Structure: This is the crossing pipeline; correctly fill in all you can. In Type/Utility check other and add either Oil or Gas next to it. Structure Material can be Steel Line Pipe. List the major contractor and secondary if required, like McDermott/Cal-Dive. Add main contact phone number for contractor rep.

Piping Change: Fill in for Hot Taps. Include new pipe info here.

Sketch/Description:

Very Important! Must have a detailed verbal picture. Repeat the Block Number and the X-Y coordinates. Add water depth, top of pipes, burial, suspension lengths, number of mats or bags, separation, angle of crossing (Show "North" direction), bottom conditions, Survey Unit of Measurement (for surveyor). Also, use Excel's drawing tools to make a small but correct drawing as shown in the example. An additional, a more complete (as in Designer)
should be attached.

Damage: Check appropriate box, and boy, I hope nothing was damaged.

Change to Piping Report Example

Similar to the Shell crossing report, the MMI report is used for all other clients. At times you may have to fill out both reports as in when one of our other clients cross or tie into a Shell system.

This report is also used for hot taps, repairs, and any other changes to our client pipelines.

Anchor and Pipeline Crossing Report Example

Fill in as much pertinent information as you can. This report gives us the contact information and parties involved should a potential incident is reported later.

Anchor Positioning Log Example

Complete for all anchors within 1000 feet of a client's pipeline or structure.  Comment as to any situations outside the norm.
