The accident statistics submitted by MMI are for the entire company. The statistics include accident experience by all personnel employed by or affiliated with MMI.

Independent contractors are included even though they are not employees of the company and are, therefore, not normally listed on an OSHA 300 Form. The accident information attached is being submitted using the OSHA 300 format but the company maintains a separate official OSHA 300 Form, which is limited to employees.

Independent contractors are listed on the attached document to assist our customers in fulfilling the MMS's SEMP program, and, in no way, does their listing confer employee status on independent contractors.
