The
Fall Protection plan shows the minimum requirements when performing work tasks
at heights greater than 6 ft from ground level. This includes work near and
around excavations. Use of guardrails, safety nets, or personal or fall arrest
systems should be used when the standard methods of protection are not feasible
or a greater hazard would be created. A fall protection work plan must identify
all fall hazards in the work area for each job and must contain the following:

for assembly, maintenance, inspection, and disassembly of the fall
protection equipment

Procedures
for the handling, storage and securing of tools and materials
|
 |
Overhead
protection for workers
|
 |
Emergency
procedures for prompt safe removal of workers

 |
If
any personnel is working in an area where he could ball and be submerged in
water then a U.S. Coast Guard approved life jacket or buoyant work vest must
be worn and at least one life saving skiff should be immediately available
|
Personnel must be trained annually
in the requirements of this program. All
training must be documented.
A qualified person at the specific
work site shall prepare the fall protection plan that corresponds with the
client’s safety manual.
If
personnel is required to work at heights greater than 6 feet above the ground
the following guidelines must be followed:

A
fixed platform is used with guard or hand rails, verified by a
competent person(s) or ... |

Fall
arrest equipment is used that is capable of supporting at least 5000
lbs static load per person and has:

a
proper anchor mounted, preferably overhead |
 |
full
body harness using double latch self locking snap hooks at each
connection |
 |
synthetic
fiber lanyards |
 |
shock
absorber |

 |
Fall
arrest equipment will limit free fall to 6 feet or less |
 |
A
visual inspection of the fall arrest equipment and system is
completed and any equipment that is damaged is taken out of service |
 |
Person(s)
are qualified to perform the work |
When necessary, a skilled person will be assigned to:

Recognize
fall hazards
|
 |
Warn
employees if they are unaware of a fall hazard or is acting in an unsafe
manner
|
 |
Be
on same working surface and in visual sight
|
 |
Stay
close enough for verbal communication
|
 |
Not
have other assignments that would take monitors attention from the
monitoring function.
|
When necessary a safety monitoring
system will be implemented by the client. All accidents and serious incidents
must be investigated, implementing changes to the fall protection plan as
necessary.
