Accidents  

Accident Investigations

A contractor / employee who experiences or witnesses an injury on the job that requires a doctor’s visit should contact their employer as soon as possible.  The manager will conduct an accident investigation during the shift the accident occurs.  The proper forms need to be filed as soon as possible.  The manager’s focus of the accident investigation should be to get as many facts as possible to help determine the basic cause of the accident.  The manager will take corrective action to remove the hazard from the workplace.  It is important to prevent this hazard in any area where a similar exposure exists.

 

MMI will assign a responsible employee to follow up all preventive actions.

 

Return to Work / Light Duty Program

If an injury results in a Lost Time Case, the company shall assign a representative to assure the individual receives appropriate care and returns to work as soon as possible.  For personnel who are temporarily not able to fully perform their duties Light Duty Work may be provided.  It is MMI's objective to return personnel to work as soon as medically permissible.

 

Contractor / Employee Responsibilities

Contractor / employees must recognize their role in safety.  That role should involve a responsible attitude for personal safety and the welfare of co-workers and contractors.  It is critical to the success of our safety program that contractors / employees trust the concept that injuries can be prevented. 

 

Contractors / employees are responsible for:

Immediately reporting every injury, regardless of severity, to a manager.

bullet

Reporting unsafe conditions and practices to a supervisor and, where possible, correcting it.

bullet

Participate in safety meetings and training provided by MMI or the Client.

bullet

Assisting in reporting and investigating incidents, injuries, and serious potential incidents.

bullet

Reviewing and complying with the contents of this and other pertinent safety manuals.

bullet

Complying with all regulatory requirements in relation to the job being performed.

 

Vehicle Incidents

If you are involved in an incident, IT MUST BE REPORTED and handled according to the following guidelines:

If you are involved in an incident, STOP.

bullet

If the vehicles pose additional danger for injury or damage, set out emergency reflectors or flares in order to protect yourself and others.

bullet

Get help for injured person(s).  Render first aid care to the extent you are trained and are qualified to administer.

bullet

Notify police and your supervisor as soon as possible by whatever means available.

bullet

Obtain necessary information at the scene to the best of your ability.  At least obtain:

Name(s) of those involved

bullet

Drivers license numbers

bullet

Vehicle license plate numbers

bullet

The name, policy number, and phone number of the drivers’ insurance companies

bullet

The name(s) of law enforcement people on the scene

                           (Names and badge numbers should be sufficient)

bullet

Obtain names and phone numbers of witnesses
